  1. Print the document.
  2. In clause 8- add two contact details for a Muslim friend/teacher that you trust will ensure that you are buried islamically.
  3. Sign the document YOURSELF, in front of a commissioner of oaths (e.g., police station, attorney, accountant etc)
  4. Let the commissioner sign and stamp the document.
  5. Make photocopies of the signed document and give to Muslim family and friends that you trust.
